Please read this form carefully. All provisions contained in this consent and waiver are conditions of participation in the Back-to-School Braids & Haircut Giveaway. A parent or legal guardian must accept the terms and sign this form before the child may participate.

If the parent or legal guardian does not agree to these terms or does not sign this form, the child will not be permitted to participate in the event.

Opening Statement

I, the undersigned parent or legal guardian, understand that my child is participating in the Back-to-School Braids & Haircut Giveaway hosted by Warriors of Christ.

I understand that this event may include hair-braiding and haircut services, bounce houses, inflatable attractions, foam-pit activities, games, food, refreshments, photography, video recording, and other recreational or community activities.

By signing this form, I acknowledge and accept all conditions associated with my child's participation.

1. Voluntary Participation

I understand that my decision to register my child for this event is voluntary. However, acceptance of all terms contained in this form is required for participation.

I understand that if I do not accept these terms or do not sign this form, my child will not be permitted to participate in the event.

2. Hair-Braiding and Haircut Services

I understand that hair-braiding, haircut, and hairstyling services may involve combs, brushes, scissors, clippers, hair products, heat-styling tools, and other hairstyling equipment.

I understand that potential risks may include minor cuts, scratches, scalp tenderness, hair breakage, skin or scalp irritation, allergic reactions, burns, or dissatisfaction with the completed hairstyle or haircut.

I authorize my child to receive hair-braiding, haircut, or related hairstyling services during this event. I knowingly and voluntarily accept the known and reasonably foreseeable risks associated with these services.

3. Bounce Houses, Inflatable Attractions, Foam Pits, and Recreational Activities

I understand that the event may include bounce houses, inflatable attractions, foam pits, games, and other recreational activities involving physical movement and possible contact with equipment or other participants.

Potential risks may include slipping, tripping, falling, collisions, entrapment, loss of balance, cuts, bruises, sprains, broken bones, head injuries, neck or back injuries, and other injuries associated with physical or recreational activities.

I confirm that my child is physically able to participate in these activities. I understand that my child must follow all posted rules and instructions provided by Warriors of Christ, event staff, volunteers, vendors, and attraction operators.

I authorize my child to participate in the recreational activities offered during the event and voluntarily assume the known and reasonably foreseeable risks associated with those activities.

4. Food and Refreshments

I understand that food and beverages may be prepared, served, or distributed during the event by Warriors of Christ, its volunteers, vendors, sponsors, community partners, or other third parties.

I understand that consuming food or beverages may involve risks, including allergic reactions, food sensitivities, choking, foodborne illness, and cross-contact with allergens.

I understand that Warriors of Christ cannot guarantee that any food or beverage will be free from peanuts, tree nuts, dairy, eggs, wheat, soy, seafood, or other allergens.

I authorize my child to receive and consume food and beverages offered during the event. I knowingly and voluntarily accept the risks associated with my child consuming food or beverages during the event.

5. General Assumption of Risk

I understand that participation in this event may involve inherent, known, unforeseen, and reasonably foreseeable risks, even when reasonable safety precautions are followed.

I knowingly and voluntarily accept the risks associated with my child's participation in the event, including risks related to hairstyling services, recreational activities, inflatable attractions, foam pits, games, food, beverages, and interaction with other participants.

6. Release of Liability

To the fullest extent permitted by applicable law, I release and agree to hold harmless Warriors of Christ, its pastors, leaders, employees, staff members, volunteers, participating hairstylists and barbers, vendors, sponsors, community partners, property owners, and activity operators from claims, demands, damages, injuries, losses, expenses, or liabilities arising from my child's participation in this event.

This release does not apply to conduct or liability that cannot legally be released or waived under applicable law.

7. Emergency Medical Authorization

In the event of an injury, illness, allergic reaction, or medical emergency, I authorize representatives of Warriors of Christ to contact emergency medical services and seek reasonably necessary medical care for my child if I cannot be reached immediately.

I understand that Warriors of Christ does not assume financial responsibility for medical services. I accept financial responsibility for any medical transportation, examination, care, treatment, medication, or related expenses provided to my child.

8. Photography and Video Acknowledgment

I understand and acknowledge that Warriors of Christ will take photographs and video recordings throughout the Back-to-School Braids & Haircut Giveaway.

Because this is a public group event, my child may appear in photographs or recordings, including individual, group, candid, or background images.

I authorize Warriors of Christ to photograph and record my child during the event. I grant Warriors of Christ permission to use photographs and recordings containing my child's image, likeness, or voice for event documentation, ministry communications, promotional materials, websites, social-media platforms, printed materials, advertisements, and future event promotion without payment or additional approval.

I understand that photography and video recording are conditions of participation and that Warriors of Christ cannot guarantee that my child will be excluded from photographs or recordings taken during the event.

9. Email and Text-Message Communications

By providing my email address and mobile telephone number and signing this form, I agree to receive administrative, informational, and promotional communications from Warriors of Christ concerning this event, upcoming events, ministry programs, church news, community resources, and related announcements.

I understand that communications may be sent by email or recurring text message. Message frequency may vary, and message and data rates may apply.

I understand that I may unsubscribe from promotional emails using the unsubscribe option included in the email. I may stop promotional text messages by replying STOP or request assistance by replying HELP.

10. Parent/Guardian Certification and Final Acknowledgment

By signing below, I confirm that:

• I have carefully read and understand this entire Parent/Guardian Consent, Assumption of Risk, and Liability Waiver.

• I understand that acceptance of all provisions contained in this form is required for my child to participate.

• I voluntarily agree to the terms of this form.

• I authorize my child's participation in the services and activities described in this form.

• I confirm that I am the child's parent or legal guardian or am otherwise legally authorized to provide consent for the child.

• I understand that if I do not agree to and sign this form, my child will not be permitted to participate in the event.
